Designated as the Social Care Network (SCN) Lead Entity for the Capital Region, Central NY, and North Country under New York’s 1115 Waiver Amendment’s SCN & Health-Related Social Needs (HRSN) Program, we are responsible for ensuring that there is a seamless, consistent end-to-end process for HRSN screening, navigation, and the delivery of HRSN and other services to thousands of Medicaid Members. - Salary range: $83,050-$95,507.50 per year, commensurate with experience
Who You Are
The
Training Specialist will develop and deliver high-quality, learner-centered instruction to promote competence, compliance, and confidence among all Social Care Network (SCN) stakeholders. This role will provide ongoing technical assistance, collect feedback to improve training outcomes, and collaborate closely with cross-functional teams to ensure consistency and compliance with program requirements.
What You’ll Do
- Develop, update, and deliver engaging training curricula for diverse SCN roles and functions using adult learning principles.
- Facilitate live (in-person and virtual) and asynchronous training sessions for internal teams and partner organizations.
- Tailor training content to meet the needs of various roles within organizations, including but not limited to Screeners, Navigators, Enhanced Health-Related Social Need (HRSN) Service Providers, finance teams, and other SCN users.
- Serve and collaborate with other team members who are subject matter experts on the IT and Data platform to support platform demos and train users on system functionality, workflows, data entry and documentation, and referral management.
- Provide ongoing technical training assistance, refresher training, and just-in-time learning support.
- Maintain and improve training materials, guides, and job aids and support optimization of the Learning Management System (LMS) platform.
- Track training and education attendance and gather feedback to evaluate training effectiveness.
- Work cross-functionally with the implementation team to ensure consistency in messaging and training practices.
- Respond to partner and staff training inquiries, offering guidance and follow-up as needed.
- Collaborate with subject matter experts to ensure content reflects current policies, systems, and operational changes.
- Contribute to continuous quality improvement efforts across the training and education program.
- Perform other responsibilities and duties as assigned.

Requirements
What You’ll Need
Education
- Bachelor's or associate degree in public health, social work, human services, health administration, or a related field (or relevant professional or lived experience).
Professional Work Experience
- Minimum two years of experience in training or instructional delivery, preferably in health or human services, social care, or public health settings.
Knowledge, Skills, And Abilities
- Familiarity with Unite Us or similar care-coordination platforms strongly preferred.
- Excellent communication and facilitation skills across diverse audiences and learning styles.
- Thorough knowledge of adult learning methods along with a commitment to developing and delivering trainings that incorporate adult learning principles.
- Ability to translate complex systems and workflows into accessible, user-friendly training.
- Strong organizational skills with the ability to manage multiple priorities and deadlines, working independently and in team settings.
- Proficiency with virtual training platforms, e-learning tools, and Microsoft Office Suite.
