Responsibilities

Conduct thorough patient assessments to determine physical therapy needs across inpatient, outpatient, and home health settings
Develop and implement individualized care plans targeting mobility, strength, pain management, and functional recovery
Collaborate with multidisciplinary teams to coordinate discharge planning, care transitions, and community reintegration strategies
Monitor patient progress regularly and adjust treatment plans accordingly to ensure continuous improvement
Maintain accurate documentation of patient care using Electronic Medical Record (EMR) systems in compliance with healthcare standards
Educate patients and their families on exercises, injury prevention, and health maintenance strategies

Experience Requirements

Doctor of Physical Therapy (DPT) degree from an accredited program required
Current and valid state licensure as a Physical Therapist
Previous clinical experience in outpatient, inpatient, or rehabilitation settings preferred (new graduates with strong clinical rotations are encouraged to apply)
Experience evaluating and developing individualized treatment plans for a diverse patient population
Strong knowledge of anatomy, physiology, kinesiology, and evidence-based treatment practices
Ability to document patient care accurately and efficiently using electronic medical records (EMR) systems
Excellent communication and interpersonal skills with the ability to educate patients and families
Strong organizational skills and the ability to manage a caseload effectively in a fast-paced environment

Pay: $40.00 - $42.00 per hour
