Responsibilities
- Conduct thorough patient assessments to determine physical therapy needs across inpatient, outpatient, and home health settings
- Develop and implement individualized care plans targeting mobility, strength, pain management, and functional recovery
- Collaborate with multidisciplinary teams to coordinate discharge planning, care transitions, and community reintegration strategies
- Monitor patient progress regularly and adjust treatment plans accordingly to ensure continuous improvement
- Maintain accurate documentation of patient care using Electronic Medical Record (EMR) systems in compliance with healthcare standards
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies
Experience Requirements
- Doctor of Physical Therapy (DPT) degree from an accredited program required
- Current and valid state licensure as a Physical Therapist
- Previous clinical experience in outpatient, inpatient, or rehabilitation settings preferred (new graduates with strong clinical rotations are encouraged to apply)
- Experience evaluating and developing individualized treatment plans for a diverse patient population
- Strong knowledge of anatomy, physiology, kinesiology, and evidence-based treatment practices
- Ability to document patient care accurately and efficiently using electronic medical records (EMR) systems
- Excellent communication and interpersonal skills with the ability to educate patients and families
- Strong organizational skills and the ability to manage a caseload effectively in a fast-paced environment

Pay: $40.00 - $42.00 per hour
