Overview
We are hiring Licensed Clinical Social Workers, Licensed Mental Health Counselors, and Licensed Marriage and Family Therapists to provide on-site services in nursing home settings. This role is ideal for a clinician who enjoys working independently, collaborating with facility staff, and supporting residents with emotional, behavioral, and adjustment-related concerns.
Clinicians will complete mental health evaluations, provide consultations, document services, and make treatment recommendations based on each resident’s needs. The work is meaningful, structured, and focused on improving residents’ quality of life.
Responsibilities
Conduct mental health evaluations for residents in nursing home settings
Provide individual counseling, supportive therapy, and clinical consultations as appropriate
Assess residents’ mood, behavior, coping skills, adjustment, and psychosocial needs
Develop treatment recommendations and care plan input
Collaborate with nursing home staff, medical providers, social workers, and interdisciplinary teams
Complete timely and accurate clinical documentation
Maintain professional standards and comply with HIPAA and facility policies
Support residents dealing with depression, anxiety, adjustment issues, grief, trauma, cognitive changes, and medical-related stressors
Qualifications
Active New York State license as an LCSW, LMHC, or LMFT
Experience working with adults, older adults, nursing home residents, or medical populations preferred
Strong clinical assessment and documentation skills
Ability to work independently and manage a caseload
Professional, dependable, and comfortable working on-site in facility settings
Strong communication skills and ability to collaborate with interdisciplinary teams
Preferred Experience
Experience with geriatric, medically complex, or TBI populations
Medicare/Medicaid documentation experience
Experience providing psychotherapy, evaluations, or behavioral health consultations
